HOW TO TAKE THE SURVEY
1. Click 'Register your facility' to get a Unique Facility Password (UFP).
2. Click 'Take the Survey' and enter your UFP Code. Go through each item and section by selecting using the three lines at top left. Once you grasp what it entails, commence with data entry. Submit at the end of each data entering except otherwise not requested.
